4/8/2024 EnchantaVaughn Approved 1.Behavior: QMHP-C arrived to the home of Ashiyah to conduct an individual counseling session. Ashiyah appeared to be energetic and jovial. QMHP-C informed Ashiyah that the focus of the session is learning effective coping and anger management skills to reduce stress when upset or angry. Mother reports when she cannot have her way she will lash out on others at least 3 times weekly with behaviors lasting up to 10 minutes in duration. 2. Behavior: Ashiyah presented cooperative behaviors throughout session as she was able to discuss challenges with her mother, but she knows how her mother is so she really don’t be listening to her or be worried about her which this behavior last on a daily basis lasting for about 20 minutes in duration. 1.Intervention: QMHP-C began the session by inquiring about Ashiyah’s current thoughts, feelings and emotions. QMHP-C processed with Ashiyah as to how she was feeling today. QMHP-C probed Ashiyah to talk about the various emotions that she experienced since the last session was conducted with QMHP-C. QMHP-C assisted Ashiyah with understanding the cause and effect of emotions. QMHP-C provided Ashiyah with role playing activities relevant to various emotions. QMHP-C probed Ashiyah to talk about ways that she can utilize her coping skills to assist her. QMHP-C assisted Ashiyah with this task by providing alternatives in various settings inclusive of home, school and community. Ashiyah and QMHP-C completed a worksheet to begin a series of self-actualization lessons. 2. Intervention: QMHP-C assisted Ashiyah with interpreting her answers as well as to the triggers that cause her to loose control of her emotions. QMHP-C concluded with a review of the concepts that were learned while providing Ashiyah with support through active listening, role playing, and summarizing what was discussed with her during today’s session. 1. Response: Ashiyah appeared to be receptive to QMHP-Cs’ interventions and services that were provided to her during the session. Ashiyah was receptive to QMHP-Cs’ intervention. She indicated during the session that she was still a little tired but she had energy. Ashiyah was cooperative during the role playing activities by providing QMHP-C with feedback to the information that was being presented to her by QMHP-C during today’s session. Ashiyah stated during the session as QMHP-C was reviewing with her different techniques on how to control her angry that she tries to use self-discipline, but sometimes she cannot control her emotions especially when she is already mad. 2. Response: She indicated that when she is really mad she will begin to cry and then she doesn’t want to hear anything that anyone has to says (indicating that she is really angry and she may slap someone).
